A $35.00 Service Charge will be applied for all returned checks.

ELIGIBILITY:  Entrants in all KGA tournaments shall be required to have an active Handicap Index at a Kentucky Golf Association Member Club at the time of entry and meet the championship's residency requirements as outlined in the 2019 Residency Policy. The current USGA Handicap Index shall be based on records effective on the posted revision date.

ELIGIBILITY: In addition to a player's agreement to abide by the Kentucky PGA General Tournament Policies, the following criteria is required.

Professional Golfers:

- All active PGA Members and Associate Members and any professional golfer, regardless of residency

Amateur Golfers:

- Either hold an active USGA Handicap Index from a Kentucky Golf Association Member Club; or

- Non-residents of Kentucky must hold a USGA handicap at a USGA Allied Golf Association
